Waiver Form: This document sets forth the terms and conditions to be adhered to by both participants and coaches during our Elite Camps. Payment Terms: Payment for the Elite Camp must be rendered prior to the commencement of the camp. Participant Waiver & Liability Agreement: I acknowledge the inherent risks associated with engaging in sports and related activities. By indicating my agreement via the checkbox provided, I acknowledge assuming all risks of injury to myself or my children/wards while participating in any activity at Toronto Futbol Events, as well as injuries sustained while on the premises. I affirm that I or my child possess the physical and mental capacity to engage in physical activities and have undergone a medical evaluation by a licensed physician within the last year. I hereby authorize camp trainers, coaches, or contracted healthcare professionals to administer preliminary treatment and arrange transportation to a local Emergency Room in the event of illness or injury to myself or my child. By consenting to this training agreement, I grant permission, either on my behalf or as a legal guardian of a minor, for Toronto Futbol Events to employ images and video footage captured during the camp for social media and promotional purposes. Refund & Cancellation Terms: Cancellation within 14 days preceding the start date of the Elite Camp will result in the full fee being incurred. Full refunds will be subject to a transaction fee imposed by Stripe or PayPal, the specifics of which may vary. Non-attendance at the camp without prior notification will result in the full fee being levied. Cancellation Due to Extreme Weather Conditions: In the event of extreme weather conditions necessitating cancellation of a camp day, parents or guardians will be promptly notified to pick up their children. Toronto Futbol Events reserves the right to prioritize the safety of participants in such circumstances. Cancellation of a Week or Entire Camp: Toronto Futbol Events retains the right to cancel a week or the entirety of a camp session. In the event of such cancellation, a full refund will be issued. Alternatively, if a week is canceled, participants may be offered enrollment in another available week, subject to availability. Modification of Coaching Staff: Toronto Futbol Events reserves the right to make changes to the coaching staff as necessary. In the event that a staff member becomes unavailable, suitable replacements will be arranged to ensure the continuity and quality of the camp experience. By assenting to this Waiver and Liability Agreement, I affirm that I have thoroughly perused, comprehended, and assent to all its terms and conditions, including the authorization for treatment provision. I affirm that I am voluntarily executing this waiver and liability agreement with full awareness of its implications, and it shall be binding upon myself, my heirs, executors, administrators, and assigns.
